Patients requesting records: Please complete the Medical Records release authorization form at the bottom of this page and hit “submit”. Please allow 7 to 10 business days to process your forms.

Following up on an existing request: If you are following up on an existing medical record request, please call 844-887-8109.

Other Medical Records Request: If you are not a patient and wish to request medical records, please fax a copy of the request to 404-497-4053.
